Abstract

Title: Investigating unintended pregnancy complications compared with intended pregnancy Background: Based on new policies to increase the population and also the reduction of access to contraceptives compared to the past, the unintended pregnancy is more likely to happen. In Iran, mothers often accept the continuation of pregnancy but during this period, they suffer from a lot of mental stress that causes many complications. The aim of this study was to compare the three common complications of pregnancy namely preeclampsia, preterm labor, and low birth weight among unintended and intended pregnant mothers. Method and Materials: This case-control study was conducted on 200 pregnant women (18 to 35 years old) who were referred for delivery to Shahidan Mobini Hospital in Sabzevar city. After checking inclusion and exclusion criteria based on the provided questionnaire, mothers were divided into two groups –unintended (100 individuals) and intended (100 individuals) pregnancy. After delivery, three complications of pre-eclampsia, preterm labor, and low birth weight were compared in two mentioned groups. All the analysis was performed by SPSS software Ver. 22 and statistical significance considered at <0.05. Results: In unintended pregnancy, mothers were older and have higher gravidity than mothers with an intended pregnancy. Preeclampsia was 5% and 1% in the unintended and intended groups respectively. Moreover, low birth weight in the unintended and intended group was 25% and 16% in turn. Furthermore, unintended pregnancy was defined as a risk factor for pre-eclampsia and low birth weight (OR: 2.5 and 7.7 respectively). Preterm labor was 12% in the unintended group and 11% in the intended group. No significant relationship was found between the time of delivery and unintended pregnancy (P=0.5). In all of the mothers, the highest prevalence of these complications was low birth weight, preterm labor, and preeclampsia respectively. Conclusions: Based on our findings, unintended pregnancy can be a risk factor for increasing preeclampsia and low birth weight in mothers. Moreover, the unwillingness to be pregnant will increase these complications. Therefore, the necessary measures should be performed to detect early or unwanted pregnancy, pregnancy assessment to determine the risk as well as precise monitoring of pregnancy and fetus to reduce these complications.
