Abstract
Background: Considering the increase of this disease and its complications, the aim of this study was to investigate the prevalence of related factors on the development of renal stones in outpatient or admitted patients in Heshamateyeh Hospital in Sabzevar, Iran in 1395. Materials and Methods: This cross-sectional study was performed on 178 kidney stones from 1 month to 14 years old referred to Heshemateyeh Hospital and outpatient clinics in 1395. Patients were examined for gastometric, uric acid, electrolytes, urinary calcium, creatinine, oxalate, uric acid, and nitroprusside screening tests for cystinuria during admission or visit at the clinic. Anatomical abnormalities of the kidney were also investigated in ultrasonography, Kub, and metabolic disorders. Finally, the collected data were analyzed using SPSS16 software. Results: The mean age of the patients was 3.56 ±3.53 years. 10.7 % of the children had a history of kidney stone, 8.34 % had a family history of stones, 5.18% had a history of urinary tract infection and 4.8% had history of drug use. 68% of the children had hematuria, 64.6% had restless symptoms, 53.4% had burning feeling, 33.7% had fever and 26.4% had abdominal pain. The mean of calcium was 10.04 mg / dL, sodium was 137 mEq / L, potassium was 4.9 mEq / L, phosphorus was 4.82 mg / dL, BUN was 17.55 mg / dL, creatinine Was to 0.54 mg / dL, and the mean calcium / creatinine ratio was 0.57. The most common metabolic abnormalities were hypercalciuria (24.2%), hyperuricosuria (12.4%) and hyperoxaluria (7.3%) respectively. Also, E. coli (15.7%) was the most common pathogen in urine culture in children under study. The average size of the stone was 42.41 ± 4.2 mm and the location of the stone in (56.7%) of the patients in the left kidney. gastometric results showed 4 metabolic acidosis and 1 person showed metabolic alkalosis with diagnosis of bartter syndrome. Finally, 5.1% of the children were relapsed. The results of the statistical test showed that there was a significant relationship between the size of kidney stones in patients and metabolic disorders (P = 0.03). Conclusions: Considering the high incidence of stones in children with risk factors, the preventive measures should be performed in children with higher risk to reduce the occurrence of stone and its complications. Since hypercalciuria is the most common cause of stones, Therapeutic measures in this group of patients are based on this metabolic disorder.
