Abstract
Introduction: Resistant to standard treatment regimens has led to unsatisfactory cure rate in Helicobacter pylori eradication. This necessitates a search for new regimens to cure Helicobacter pylori infection. The aim of this study was to evaluate the efficacy of Levofloxacin-containing quadruple therapy and to compare it with that of standard triple therapy as the first line for the Helicobacter pylori eradication. Materials and Methods: This is a clinical trial, nonprobability sampling study. Ninty eight eligible patients referring to the gastroenterology clinic of Vasei Hospital in Sabzevar were chosen purposefully. Then, pateint randomly were divided every other two groups. First group treated by Levofloxacin 500 mg bid, Bismuth subcitrate 120 mg qid, Amoxicillin 1 gr bid and Pantoprazole 40 mg bid for 2 weeks. Also second group will be treated by Clarithromycin 500 mg bid, Amoxicillin 1 gr bid and Pantoprazole 40 mg bid for 2 week. Eradication check was performed with stool antigen test, after 4 weeks the discountinuation of therapy. Data were analyzed by SPSS and p-value<0.05 was considered significant. Results: In this research, seven patients were excluded from the study. Ninty one pateints were consisted of 45 people with first group, as well as 46 people with second group . Differences in age, sex, peptic ulcer were not significant between two groups (p=0.117, p=0.329, p=0.935, respectively). The eradication rate was 88.89% for first group and 78.26% for second group, but difference was not statically significant between two groups (p=0.172). There were no significant differences among these therapies in the terms of the rate of tolerance (p=0.162). Pateints in two group did not differ significant in terms of side effects such as diarrhea, nausea, headache, neuropathic pain, and constipation. Discussion: This study indicates that Levofloxacin-containing quadruple therapy has no significant advantage to the first line versus standard triple therapy in Helicobacter pylori eradication, but that can be as the first line for the Helicobacter pylori eradication, especially high Clarithromycin microbial resistance rate was reported in the current studies.
