Abstract

Abstract: Comparison between validity of transcutaneous and serologic bilirubinometery in neonates under phototherapy admitted in vaseyi hospitsl of sabzevar Background: Neonatal icterus is one of the most common causes of neonatal admission. The vival method of measuring serum bilirubin is neonate s foot sampeling.one of the methods which is used nowadays for bilirubin screening is transcutaneous bilirubinometry .the aim of this study is the evaluate the validity of this method during phototherapy reducing the time and complication of blood sampling. Method & Materials: in this study 200 neonate which were admitted in vasei hospital of sabzevar were studied.in admission time both serologic and transcutaneous sampling of sternum and frontal were done .site were shielded by 6-layer gauzes and serologic and trans cutaneous sampling was performed daily. Result: data were analyzed by two methods of kappa and ICC. INTERNAL CORRELATION COEFFICIENT of serologic and transcutaneous bilirubinometry of frontal was fair(0.3) in the first day, poor (0.07) in the second day and moderate in the third day(0.48) and forth day (0.61). ICC of serologic and transcutaneous bilirubinometry of sternum was poor in the first day(0.17) and second day (0.04) , fair in the third day (0.34) and moderate in the forth day(0.5).In the kappa method the kappa and the weighted kappa between serologic and transcutaneous bilirubinometry of frontal were poor agreement (0.62) and moderate agreement (0.84) respectively for the first day and the poor agreement for the second daye (0.08 -0.23). The kappa and the weighted kappa for serologic and transcutaneous bilirubinometry of sternum were moderate agreement (0.51 – 0.60) respectively for the first day , and moderate agreement (0.41) and poor agreement (0.15) respectively for the second day. Due to lack of data, we were not able to use kappa method to analyze the result of the third and forth day. Conclusion : based on the analyze correlation and agreement between two test are not satisfactory to consider transcutaneous bilirubinometer as a complete alternative for serologic evaluation during treatment with phototherapy . although it might be useful for screening.
