Abstract

Title: Comparison of the outcome of carpal tunnel release surgery in patients with moderate to severe involvement Background and Aim: Carpal tunnel syndrome is the most common focal mono neuropathy due to nerve compression. This syndrome is a collection of sign and symptoms that arise from median nerve compression when passing through the carpal tunnel. Incident age is estimated to be between 30-60 years old، about 10% of adult women and 1% of men with carpal tunnel syndrome. Our aim was to compare the result of carpal tunnel surgery in patients with moderate to severe involvement based on clinical manifestations in Sabzevar Emdad Hospital. Method: This cross-sectional study was performed on 150 patients with carpal tunnel syndrome who were referred before surgery and two months after surgery in Sabzevar Emdad Hospital for six months.Patients were devided into two groups of 75 with moderate and severe involvement based on symptoms clinical examination and electrodiagnostic tests.Tinel & phalen tests performed before and two months after surgery for patient. The Boston questionnaire was also filled up by patients before and two months after surgery.The statistics were analyzed using spss software. Results: Of 150 patients examine, 114 (76%) were female and the mean age of the patients was 54.26 years.The frequency of people with a free job (51.37%) was higher than those employed and housewives. In patients with moderate wrist involvement، the atrophy of the thenar muscle was less and more minor، and therefore gave a better response to the treatment and return of the natural neuronal pulses of the median. Before surgery، it was felt that around one-third of the population was above the average، after which the dispersion varied significantly after surgery، with nearly 80% of the patients reporting insignificantly lower than the mean level. In both groups، moderate and severe wrist involvement improvement was observed، but in patients with moderate conflict, the change in outcome after surgery was higher. Conclusion: improvement observed in the two groups of patients with moderate and severe involvement was somewhat similar, but the rate of improvement in moderate patients who expressed pain, paresthesia and atrophy as the main complaint was more than those with severe
