Abstract

Introduction: The most common cause of death in Iran and the world is the ischemic heart disease. Acute coronary syndrome, which includes unstable angina, myocardial infarction with ST segment elevation and myocardial infarction without ST elevation. One of the most important medications for the treatment of ischemic relapses is anti-coagulant drugs such as heparin. The aim of this study was to determine the comparison of heparin with an approximate dose of 2500 units every three hours with using the conventional method, it means continuous intravenous heparin infusion based on weight. Methods: This clinical trial study was performed on 60 patients with acute coronary syndrome in two groups of 30 patients that randomly assigned to one of the groups. Alternating Injection were considered for the intervention group and continuous infusion for the control group. Patients in both groups received heparin therapy for 48 hours after admission and ,to control the complications and prevent the APTT falling, APTT was taken from patients each 6 hours. At the end, the results of the test were recorded in the checklist for statistical analysis. results: According to P> 0.05, the Kolmogorov test was not significant.it means, the distribution of data was normal. Regarding to the normal distribution of data, independent t-test was used to evaluate the mean of APTT in both groups of alternating Injection and continuous infusion with syringe pump. After independent t test, according to the test results to determine the consistency of variances, one of the assumptions of independent t test, which assumes the consistency of variances, is called the Levine test used due to the sig = 0 427 in APTT1 and sig = 0 195 in APTT8, the results of the row of equivalence of variances were considered. Then, to determine whether there is a significant difference between the two groups of alternating Injection and continuous injection with a syringe pump or not The Sig (2 tailed) section was considered. Due to Sig (2tailed) = 0.061 in APTT1 and Sig (2tailed) = 0.882 in APTT8, there was no significant difference in mean APTT dependent variable in both groups. Conclusion: In general, this study showed that according to the results of statistical analysis with independent t-test, there was no significant difference in the mean of the relative level of thromboplastin in the two groups of continuous infusion with syringe pump and intermittent intravenous injection which means the use of heparin in both continuous infusion and intermittent infusion had almost the same effect on APTT and in terms of possible complications, none of the patients in either group experienced a complication of heparin. For this reason, it can be suggested that we can use falternate infusion instead of continuous infusion.
