Abstract

The assessment of the therapeutic effect of human amniotic membrane (HAM) dressing on the graft donor site healing in patients with burns compared with standard therapy. Background: Burns are the common and most devastating forms of injury that could be jeoprdize physical and mental health.Today the standard treatment for deep partial thickness and full-thickness burn is early excision and grafting. But the limited major factor is unavailability of sufficient donor site in order to provide the skin that be sufficient for graft. In this study, we try to provide at least necessary time for healing of donor site by using of HAM, then we use the achieved tissue in the place of before donor site in order to have a new skin graft.. Methods and materials: In this double blind clinical trial,we consider 35 members who came to burn parts of Vasei hospital. they have 2rd and 3rd degree burns and their ages were Between 2-60, and they need to have surgery and autologous skin grafting. After aseptic procedures in the operation room and cutting skin graft, donor site divided to two equal parts and randomly divide to two separated groups, Intervention and control group, And each group rercive the necessary treatment.then we ask from 3 colleage of the project, who aren’t aware of devoted groups of donor site, to estimate the level of wound healing and comparing them with each other. The General Surgeon and dermatologist by using the provided chek list and observation examined photographicmorphometry in 10th and 20th and 30th days of post operation and scoring them according to chek list. The biopsies were analyzed too.it was done by special permisson of patiant, limitedly in differents days. The data analyse with spss software. Results: In this study %62.9 of the participant were male also %74.3 were over 29 years old. The average of final score in intervention group was 14.60 and control group was 14.14,that isn’t significan (p=0.122). and using of HAM has no therapeutic effect on the interventiona group in compare with control group.on the other side, the average of epithelialization score in intrvention group was 7,14 and in control group was 6.77 that is significant (p=0.021). it ssems that HAM can improve epithelialization only. Conclusion: According to current study,the HAM only cause to improve epithelialization on donor site and it has no effect on other cases and using it on donor site for healing has no advantage on standard treatment and it has no logical benefition and increase the cost of treatment.
