Abstract

Frequency of ESR and CRP in Patients with Acute Appendicitis Referred to Emdad Hospital, Sabzevar Background: Acute appendicitis is one of the most common surgical emergencies which its diagnosis is based on medical history, physical examination, and para-clinic data. However misdiagnosis is relatively common. Therefore, we aimed to evaluate the statues of inflammatory markers in diagnosis of acute appendicitis. Method: in this case control study 210 patients, who were suspected for acute appendicitis; were enrolled. The study was conducted in Emergency ward of Emdad Hospital during 2017. A blood sample was obtained before appendectomy for assessment of ESR and CRP. All tissue of appendices were evaluated pathologically. After determination of results of ESR, CRP, WBC, and pathologies, data were analyzed by SPSS20. The level of significance was considered p≥0.05 Results: Overall, 210 were enrolled. Mean age of patients with acute appendicitis was 28.25 (4-88 years). Mean WBC count of patients was 11259±3839 (4100-20000). Frequency of ESR and CRP in patients with pathologically confirmed acute appendicitis were 61% and 60%, respectively. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of ESR in diagnosis of acute appendicitis were 60%, 59.05%, 59.43%, and 59.62, respectively. Sensitivity, specificity, PPV, and NPV of CRP in diagnosis of acute appendicitis were 60%, 89.52%, 85.14%, and 69.12, respectively. Conclusion: The results of present study suggest that CRP could be used specifically for diagnosis of acute appendicitis even with qualitative evaluation. In fact this test is useful in diagnosis of this medical condition, although could specifically rule out it. However as unique test could not be referenced. Thus, using CRP as a para-clinic test plus Alvarado score may be very beneficial in diagnosis of acute appendicitis. We recommend to assess the level of CRP before appendectomy. Regarding ESR, the results of present study showed a low sensitivity and specificity, therefor we recommend against its routine evaluation in patients with acute appendicitis Keywords: acute appendicitis, Differential diagnosis, ESR, CRP
