Abstract

Title: Survey the addition of tadalafil to tamsulosin in the treatment of acute urinary retention in patient with benign prostatic hyperplasia. Background and Purpose: Acute urinary retention (AUR), is one of the most significant complications of benign prostatic hyperplasia (BPH).Until now, standard treatment including catheterization and use of alpha blocker drugs.Tadalafil is a phosphodiesterase type 5 inhibitors which is used in the treatment of the erectile dysfunction. Recently seen that this drug in the treatment of urinary symptoms caused by BPH also plays a role, especillay the rapid onset within a few minutes. The aim of this study, survey the addition of tadalafil to tamsulosin in the treatment of acute urinary retention in patient with benign prostatic hyperplasia. Materials and Methods: This is a clinical trial, randomized, double blind study. Eighty patient with AUR related to BPH have been divided into two groups of 40 people and randomly assigned to receive either 0.4 mg Tamsulosin plus placebo or 0.4 mg tamsulosin plus 10 mg tadalafil for seven days. On the same visit, the catheter was removed and ability to void in each group was assessed in 24 hours and a week later. Data were analyzed by SPSS-21 and K2, T test and p<0.05 was considered significant. Results: Differences in age, the volume of urine retention, history of drug use, LUTS and Previous AUR was not significant between two groups (p=0.619, p=0.149, p=0.501, p=0.284, p=0.371). After catheter removal, 23 patients (57.5%) in placebo group and 26 patient (65%) in tadalafil group voided successfully at 24hours (p=0.491). After 1 week, 29 patient (72.5%) taking placebo and 26 patient (65%) taking tadalafil could void yet but difference not statically significant between two groups (p=0.041). Conclusion: It seems that addition of tadalafil to alpha-blockers has no significant advantages to improve AUR related to BPH versus tamsulosin alone.
