Abstract
Introduction: Early and accurate diagnosis of perforated appendicitis is essential to avoid complications and choosing appropriate treatment. The aim of present study was determination of the role of total bilirubin and C-reactive protein (CRP) levels in predicting perforated appendicitis before surgery. Material and methods: A total of 208 patients older than 15 years who were refered to Sabzevar Emdad Hospital with right lower quadrant abdominal pain and suspecious history and physical examination of acute appendicitis enrolled in this study. Patient's information includeing name, registration number, date of admission, age, gender, previous history of liver disease as well as fever, symptoms and results of preliminary examinations were documented in a researcher made check list. For all patients serum bilirubin and CRP levels were measured and results were also documented. After surgery, intraoperative macroscopic findings as well as pathology results were recorded. Results: Upon study population, 55% of patients were male and 45% were female with a mean age of 27 years. The perforated appendicitis based on pathology was seen in 27% of patients and was more common in males (63%) rather than females. The results of the study showed that bilirubin and CRP levels were significantly higher in perforated cases. (P = 0/000) Discussion and Conclusion: According to the results of this study, patients with symptoms of appendicitis and perforated appendicitis were more likely to have hyperbilirubinemia and high CRP. Thus, measuring total bilirubin and CRP levels can be used in conjunction with other laboratory parameters used in the early diagnosis of perforated appendicitis.
