Abstract

Background: shivering is an important and unpleasant complication of anesthesia which is reported in 5-65 and 55 percent of general and neuraxial anesthesia respectively. in addition to patient discomfort and medical monitoring such as blood pressure, electrocardiogram and oxygen saturation, shivering has other adverse effects such as increasing oxygen demand and carbon dioxide production, increase in sympathetic tone, cardiac output, heart rate, blood pressure, ocular pressure and also causes lactic acidosis as well as surgical site pain. Long term adverse effects are increase in myocardial ischemia and infarction, delaying wound healing and increase in pre-surgical mortality rate. Method:This double blind prospective randomized clinical trial study was approved by Sabzevar University of medical Sciences ethic committee. ASAI pregnant patients of 15-40 years old who had uncomplicated pregnancy and were candidate for cesarean section with spinal anesthesia enrolled in this study after filling an informed contest. Every patient was monitored for blood pressure, blood oxygen and heart rate before taking spinal anesthesia. Patients were randomly divided in to two 40 groups naming as P and C groups. Group P received 20mg(2cc) pethidine and group C received 2cc of sterile water both intravenously. Heart rate, blood pressure and blood oxygen were recorded each 5 minute before and after anesthesia. Results: Mean times of labor were 30 and 25 minutes for P and C groups respectively which was statistically significant(P value:0/045). Four patients in group P and 14 patients in group C had nausea and vomiting (P value:0/007 ). Group P showed significantly lesser shivering. Grade 2 and 3 shiverring was not seen in group P and grade 1shivering was also lesser than other group(P value<0/001). Conclusion: according to this study, using intravenous pethidine can appropriately reduce shivering in patients undergoing spinal anesthesia without causing specific side effects.
