Abstract

Introduction: Background: Episiotomy is one of the most common interventions in obstetrics. Delayed healing and infection may lead to adverse maternal outcomes or even maternal death . Material and methods: This double blind randoised clinical trial was designed to evaluate the effect of gentamicin solution on the healing and pain reduction of episiotomy inscision. One hundred and fifty primy parous women who were reffered to Shahidan Mobini Hospital, Sabzevar, Iran for labor enrolled in this study after filling an informed contest. Womens who filled our inclusion critera were devided to 3 groups. After complete cervix dilation a medio lateral episiotomy after performing local lidocain (2%) injection (5ml) at perineal region. before episiotomy repair, the insiscion site was whashed by 80ml gentamicin solution in gentamicin group and 100ml of normal salin in normal salin group. The control group didn’t recieved any washing. Women were evaluated for surgical site redness, discharge, fever (greater than 38 cellisius degree), tenderness, edema and warmth. The examination was performed in litotmy position by both researcher and the gynecologist which were blind to the study protocol. Results: The mean age of particpants, gestational age and body mass index were 24 years old, 39.53 weeks and 28.5 respectievly. There were not any diffrence between study groups in case of labor stages duration. The episiotomy length was not significantly different among study groups. Episiotomy site pain was significantly different among study group in the 10th day in The control group. Also, the redness and perinea edema was significantly higher in control group. During the study period there was not any significant difference between study groups in case of incscion gap. Also, the browsiness was not significantly diffrent among study groups during the study period. During the study period, there was not any significant different among study groups in case of inscisional discharge. Conclusion: Application of gentamicin solution on episiotomy insicion site is effective in pain and inflammation reduction after the third day. The distance btween the insicion ridge, bruiness and discharge were not different between study groups. While the gentamycin effect was prominnat on in every aspect of wound healing except distance btween the insicion ridge, bruiness and discharge, we recommend gentamicin solution use reduction of post episiotomy pain and discomfort.
